The value of a five-stage model is not that it is provably how any system works — nobody outside those systems knows that. It is that it separates five distinct ways of being invisible, each with a different remedy.
Discovery fails#
The source is never found. Usually crawlability, or a page nothing links to. Fix the plumbing before anything else.
Understanding fails#
The source is found but the entity is not isolated — the name varies, or the subject is implied rather than stated. Fix the wording.
Verification fails#
The claim is stated but nothing supports it. Assertion without explanation reads as a claim to be checked, not a fact to be used.
Association fails#

Why the Model Is Kept Simple#
Real retrieval and generation pipelines are far more complicated than five boxes, and any diagram that claims otherwise is selling something. The model earns its place only as a diagnostic: when an entity is not being named, it gives you five places to look instead of one vague conclusion that the content needs to be better.
